SMS Terms and Conditions

Upon booking an appointment with us, patients consent to receive SMS messages and phone calls from ReFocus Eye, our affiliates, or our duly appointed vendors. We use SMS messaging and phone calls for several functions, including existing appointment reminders, requests to confirm, billing notifications, etc. Patient mobile information will never be shared with or sold to third parties/affiliates for marketing/promotional purposes, aside from reminders to book overdue eye exams/appointments.

1. SMS Messaging Service

By providing my phone number, I consent to receive SMS text messages from Refocus Eye Health: Hamden for appointment reminders, overdue eye exam notifications, and general two-way communication. Msg frequency varies. Msg&data rates may apply. Reply HELP for support. Reply STOP to opt out.

2. Message Frequency

You will receive up to 0-6 messages per month, depending on appointment frequency

3. Message and Data Rates

Message and data rates may apply based on your mobile carrier’s terms.

4. Privacy Policy

Your information will be handled in accordance with our Privacy Policy.

5. Opt-Out Instructions

You can opt-out at any time by replying “STOP” to any SMS message. Reply HELP for support. You may also contact us directly at 203-597-5448.

6. Liability

We are not responsible for any charges, errors, or delays in SMS delivery caused by your carrier or third-party service providers.

By opting in upon booking, you confirm that you are the owner or authorized user of the phone number provided and that you are at least 18 years old.

Please Note: Many insurance deductibles reset as of January 1st. Our practice requires payment for all deductibles, copays, non-covered services, and any outstanding balances prior to your appointment at the time of check-in.
